Eligibility

  • Frail, aged & younger people with a disability & their carers.
  • Families experiencing stress or illness which threatens the adequate functioning of the household (eligible for short term services only)

Referrals

Anyone can refer to Home Care Service. Self, family, and health professionals are the usual means of referral.

Home Care Services

Tasks may include: Cleaning, laundry care, and ironing. In addition the staff can do shopping, banking, food preparation, letter writing, etc.

Personal Care

  • Assistance with showering, grooming, dressing, getting in and out of bed, and social support.
  • District Nurses only attend to personal care when there is a medical need to do so.

 

Respite Care

Assistance with regular respite. Limited evening, overnight and weekend respite care is available through HACC and other programs, contact Coordinator for further details.

For extended periods of respite care, please give as much notice as possible.

Home Maintenance

Available to residents who do not have access or other means of assistance with minor home maintenance.

Tasks May include: annual cleaning windows, garden services (max 2 hours every 3 weeks), changing of light globes, limited minor repairs.
Installation of aides eg. Rails, safety and security equipment.

Emergency (Short Term) Services

Full range of HACC services are available for a short time for eligible people.

Meals On Wheels

Home delivered meals are available 7 days per week.
People needing meals do not require a doctors referral, but will be assessed for eligibility
.

Disabled Parking Permits

Application forms are available from UMH&CS reception or HACC Coordinator.

Your doctor will need to fill out sections of the form to establish your eligibility.
